This form should be completed by members of the J Sainsbury Pension & Death Benefit Scheme or J Sainsbury Executive Pension Scheme, who remain working for Sainsbury's but wish to leave the Scheme. You cannot re-enter this scheme at any time and you will be giving up a valuable benefit.

This form should be completed if you are a member of the J Sainsbury Pension & Death Benefit Scheme or J Sainsbury Executive Pension Scheme, and you remain working for Sainsbury’s but wish to change the section of the Scheme you are a member of. You cannot re-enter the section of the Scheme you are leaving therefore independent financial advice may be helpful to you before making a decision.
